Once you have a Name, you may need an Employer Identification Number (“EIN”), which is obtained by filing form SS-4. (If you will have payroll or are a corporation or partnership, you must have an EIN) You can call the “tele-file” number (866-816-2065, Philadelphia, PA) and get the EIN over the phone, but you will need to have the form completed before you call. You can go to the site below and fill out the SS-4 form and then print it.

Oklahoma Employment Security Commission 

If you will have employees you will need to file OES-1 Oklahoma Employment Security Commission – Employer Status Report which can be obtained from the following web-site [FAX 405/524-6081]. You will need to copy and paste the address below:

Oklahoma Tax Commission - Business Registration Packet – This package needs to be filed as soon as you have an EIN or need a State Sales Tax certificate. Pages 1-2 are required to be completed. Other sections may be required depending on whether or not you will be withholding Oklahoma Income Tax, collecting state sales tax, are a manufacturer or will be conducting other reportable activities. This package can be obtained at the website below and filed at the Ardmore Oklahoma Tax Commission office on the third floor of First Bank & Trust (301 W Main). It takes about 45 minutes, but any and all issues will be resolved on the spot. (A corporation must have already registered with the Secretary of State before going to the Tax Commission office.)

Completion of the Business Registration package will cover the initial Franchise Tax, Sales and Withholding Tax registration.
